Debbie Richter RN, BSN and Sarah Ross Moberg RN, BSN, with certified home health aide Peggy Garst are the skilled home health team. They work together to provide a variety of services to the elderly, disabled, and those in need of home care living in or near Lindsborg. As Lindsborg Community Hospital employees, the home health team offers the additional benefit to patients and their families of improved continuity of communication between physicians and other health care providers.
We believe healthcare is the highest form of service! We take pride in providing the best home care available to all of our patients.
Referrals and Admission:
Anyone may refer a client to home care services. To receive registered nursing services, the client's doctor will be involved in the care. (The client's home must be adequate for safe and effective care.)
Home health aides are routinely available Monday and Thursday.
Registered nursing services are provided on a part-time basis.
Registered nurses provide private pay services such as medication set-ups, assessments, vital signs monitoring and simple wound care. Home health aides provide private pay services such as bathing assistance and nail care. Home health aides can provide in-town errands and well-being checks.
CareLink® Personal Emergency Response
CareLink® provides the ability to quickly and reliably summon help at the touch of a button 24 hours-a-day, 7 days-a-week to anyone living at home. The support center is trained to quickly and accurately determine what level of assistance is needed and make certain help arrives as quickly as possible. A simple enrollment form is all that is needed to start the service. Choose between the popular 7100 Series unit, offering crystal clear and reliable two-way voice communication or the 8100 Series unit, which also includes the ability to add daily reminder messages. Reminder messages may include: Remembering to check blood sugar, respiratory treatments, eye drops or even to feed a pet.
CareLink® equipment is reliable and provides excellent voice quality, offering hands-free, two-way voice communication throughout the entire home. Weighing less than half an ounce, the waterproof pendant is the smallest unit of its kind. It can be worn as a pendant, wristband or belt clip. For assistance, simply press the pendant or emergency button and the Healthcom attendant will be notified and summon help. Adaptive devices are available for those who have difficulty or are unable to press the button.
Medication Management Systems
According to the Center for Medicare and Medicaid Services (CMS), 30% of Medicare hospitalizations are a direct result of medication mismanagement. While dangerous consequences result from missing medicine, those who suffer from heart disease are especially vulnerable. CareMed® represents our collection of medication management services that can help with the daunting task of sorting and taking medications as prescribed. MedReady+™ a reliable 28-dose solid medication dispensing system with live 2-way monitoring through the CareLink® Support Center.
This simple, compartmentalized medication system provides an easy way for persons to be reminded to take medications as prescribed by their physician. Available with or without monthly monitoring, the system can improve medication compliance to over 98%. Capable of dispensing medications up to 4 times a day for 7 days, it can send email or text messages to caregivers to keep them informed when medications are taken (or missed).
