Welcome to the Family Health Care Clinic! We are focused on our goal: to provide quality medical care for you and your family -young and old--in a personal, professional way.
Fourteen exam rooms are staffed by board certified physicians Benjamin Dolezal, MD, Wendy Dinkel, DO and Dr. Luke Rosebraugh, MD. Also staffing the rooms are Advanced Practice Practitioners Kelsey Swisher, PA-C and Greg Lindholm, PA-C who provide primary care services. Jade Banning, PA-C, is a member of our team and works alongside Greg Lindholm in our Medically Supervised Weight Management program.
Our Newest Service
Meet Sarah Ross Moberg, RN, BSN, our Chronic Care Management nurse. If you have Medicare or Blue Cross Blue Shield and live with two or more chronic conditions like arthritis, diabetes, depression or high blood pressure, Sarah can help you manage your chronic conditions so you can spend more time doing what you love.

Some of the things that Sarah helps her patients with include:
- Home Medication set up either in your home or in the clinic
- Understanding your health conditions including instructions from your doctor
- How to manage symptoms at home, between doctor’s visits
- Help in finding resources to help you stay in your home as long as possible (for example a fall alert monitoring system)
